Entry fees:
U9 & u10 - $450.00
U11-U18 - $500.00

Application Deadline:
April 27th, 2012

Format:
USYSA rules, subject to certain modifications.  Three game minimum, U9 & U10 will play 8v8.  All others will play 11v11.

Awards:
Individual medals for 1st and 2nd place.

Check In:
All teams must have a representive at the mandatory check in May 18th at the Doubletree Hotel, 6pm-8:30pm.

Representatives must have USYSA player laminated player cards, state or Club approved roster and approved guest player forms.

Teams participating from outside Region IV will need to provide travel papers.

All players must have approved medical release forms.
